Your Beneficiary

Naming Your Beneficiary

On your enrollment or application form, you will need to name your beneficiary, the person or persons who will receive benefits if you die. You may name anyone as your beneficiary. Your beneficiary designation will apply to your Basic, AD&D and Optional Group Term Life Insurance unless you indicate otherwise in writing.

If you do not name a beneficiary or your beneficiary has preceded you in death, the benefit will be paid to the executor or administrator of your estate, or in accordance with the other options described in your certificate.

Changing Your Beneficiary

If you wish to change your beneficiary after your initial enrollment, you may do so by completing a Change of Beneficiary form. This form is available from your Benefit Liaison or the Benefits Management Office. You can return the completed form directly to the Benefits Management office or to Bankers Life and Casualty. The change of beneficiary will take effect when your request is received and processed by Bankers Life and Casualty, during your lifetime.
