Freedom of Information requests should be printed or typed and include the following information: the name, mailing address and daytime phone number of the requester; and a description of the records being requested. Requests can be mailed, faxed or dropped off in person.
The Department of Aviation maintains the following categories of records:
FINANCE:
- documents pertaining to capital finance projects:federal grants; airport revenue bonds; passenger facilities charges, including applications, airline comments and FAA approval letters; capital finance approval from the airlines for O'Hare Airport; capital finance approval from the airlines for Midway Airport; and vendor payment vouchers for capital projects;
- documents pertaining to operations and maintenance:payment records and vouchers; proposals from vendors; yearly budget reports; invoices; sub-order release forms; Chicago Accounting and Procurement Systems (CAPS) reports; and reports regarding the receipt of goods and services;
- contracts pertaining to capital projects and operations and maintenance: all purpose requisition forms; specification documents; modifications and amendments to existing contracts; and reports regarding MBE/WBE participation;
- documents pertaining to revenue: statistics pertaining to jet fuel flowage at Midway Airport; summary of gross revenues for rental cars; summary of accounts receivable; summary of outstanding accounts receivables; and invoice registers;
CONCESSIONS:
- documents pertaining to license agreements: license agreements; intergovernmental affairs summary; ordinance; letters of credit; insurance certificates; map location of all concessions; contractor's affidavits; request for qualification information
