Freedom of Information requests can be submitted on the attached form below. Use of this form isn't mandatory, but all requests should be printed or typed and include the following information: the name, mailing address and daytime phone number of the requester; and a description of the records being requested. If you are seeking permit or violation records for a specific building, you must include the address of the building. Requests can be mailed, faxed or dropped off in person.

The Department of Buildings maintains the following categories of records:

• building permit records

• building inspection records for conservation inspections, heat inspections and technical inspections, including boiler, electrical, elevator, iron, new construction, plumbing/water, refrigeration and ventilation

• trade licensing records for stationary engineer, crane operator, electrician, elevator, apprentice plumber, journeyman plumber, plumbing contractor and mason contractor

• Fast Track demolition files

• building registration records

• exterior wall ordinance records and required filings

• departmental personnel and payroll records

• departmental budget records
